IF YOU REQUIRE ANY INFORMATION ON THIS WEBSITE, IN A DIFFERENT FORMAT TO HELP WITH ANY SENSORY IMPAIRMENT, PLEASE DO LET US KNOW. Thank you
We would like to know how we can improve our service to you and how you perceive our surgery and staff.
To help us with this, we have already set up a Patient Participation Group (PPG) which was introduced September 2011. If you would like to join this group, please contact the Practice Manager on 01924 866915 or email firstname.lastname@example.org
We are also considering setting up a virtual patient representation group, so that you can have your say. We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.
We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.
If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.

If you prefer, you can download the sign up form as a pdf document, print it out, complete it and return it to the practice.

We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.
Many thanks for your assistance

The practice and patient representatives meet a minimum of twice yearly or more frequent if required or appropriate. We also email the representatives on an ad hoc basis, to canvas advice or share information that cannot wait until the next planned meeting.
One of our patient representatives has kindly offered to be responsible for a notice board within the surgeries, as a way of sharing information with all our patients. The plan is for her to regularly update the board with information of interest and to receive feedback to share with the practice and / or group, to promote 360 degree sharing of information as best possible.
If you have questions or feedback please feel free to speak with any member of the group, or contact the Practice Manager on 01924 866915 or email email@example.com
Crofton and Sharlston Medical Practice are part of a network of practices know as Trinity, who meet regularly to share ideas and services where appropriate. Practices within this network include ourselves and also
Trinity Health Centre
Maybush Medical Practice
The network have held patient group meetings inviting representatives from each individual practice to share information about things that are going on at network level, giving examples of how the practices work together and share resources and services offered to all patients registered with any of the above individual practices.
If you would like to know more about this, please feel free in speaking to a patient representative of your practice group or the practice Manager.
